If you complete the forms at home please arrive 15 minutes prior to your scheduled appointment time so we can register your child and verify your insurance. If you are unable to complete the forms at home, please arrive 30 minutes prior to your appointment time and our staff will provide the forms for completion in the office.

To provide the best care for your child we like to review your child’s previous records. You may print the medical records release form and send it to your child’s previous doctor or we can send it from our office.
