Insurance
Health Insurance and medical billing can be confusing. Insurance benefits, deductibles, copayments, referral and other requirements vary greatly from plan to plan. These benefits are selected by you or your employer group. While understanding your insurance coverage is ultimately the responsibility of the family, our staff is available to help you understand your coverage and meet your insurance carrier's requirements.
Our office participates in several insurance plans. The following is a list of the plans with which our office participates. This list changes from time-to-time and not all of the insurance panels are currently open to accepting new patients. Please review your insurance coverage with our staff when scheduling your appointment and notify our staff of any changes to your insurance coverage.
Billing
Insurance coverage varies greatly from plan-to-plan and our staff will do their best to help you understand your coverage.
All copayments are due at the time services are rendered and patient accounts must be current and up-to-date prior to being seen for preventative services.
We accept cash, checks, VISA, Mastercard and Discover.
There is a $25.00 fee charged for returned checks.
Form Fees
There is a $10.00 fee for the completion of forms such as school forms, daycare forms, camp forms, medication forms, etc.
Medical Record Fees
You may want to check the Patient Portal to see if the information you are requesting is available to you on the portal. If you need a copy of your child’s medical record, please complete an Authorization to Release Medical Records. Copies of medical records will be made at a charge of $.50 per page. Our office does not charge a medical record copying fee for records being transferred directly to another health care provider.
